I confi rm that I consent to the gathering and processing of the personal data that I provide in my application for membership of the Society of Friends of the Jewish Museum in Prague (“the Society”). This consent shall be for the period of my membership. The personal data in my application may be gathered and processed by the Jewish Museum in Prague only for the purposes of keeping its register of members, offering benefits associated with membership in the Society and presenting the Society’s activities.

I am aware that the provision of the above personal data is voluntary. This data may be gathered only to the extent necessary for the stated purposes, is not intended for any third party and may not be made available to anyone else. I am also aware that I am entitled to request the Office for Personal Data Protection to take remediation measures in the event of a breach of the obligations of the Jewish Museum in Prague as personal data administrator pursuant to the Protection of Personal Data Act No. 101/2000, and, further, that I am entitled to request the Jewish Museum in Prague to refrain from any action in breach of said law, to rectify the resulting state of affairs, to provide an apology or other satisfaction, or to rectify, supplement, block or remove the personal data in question and, in the event of a breach of human dignity, of personal honour, of reputation or of the right to protection of reputation, to pay compensation.
